Michigan 4-H Youth Code of Conduct & Authorization Forms

The Michigan 4-H Youth Code of Conduct and authorization forms are required to participate in the MSU Extension 4-H program. Forms are signed in 4-H Online by participants and/or guardian during enrollment in 4-H Online annually.
This form includes the Michigan 4-H Youth Code of Conduct, Evaluation Acknowledgement, Media Release and Medical Treatment Authorization.
