Chronic Disease Prevention and Management

Education Provided in Allegan, Kent and Ottawa counties
PATH is for people living with chronic conditions such as diabetes, heart disease, asthmas, arthritis and chronic pain. Participants learn skills so they are better equipped to face the daily challenges of living with chronic conditions. Two specialized PATH workshops are also available: PATH—Diabetes and PATH—Chronic Pain.
This workshop consists of eight, two-hour sessions aimed at reducing the fear of falling and increasing activity levels and confidence among older adults. Throughout the course, participants establish goals for increasing activity, identify modifications to their environment to reduce fall risk, practice strength and balance exercises and learn from each other.
