Forms & Requirements

Student Responsibility

Students are responsible for assuring their program plan requirements are met.  Learning these and having clear communications with your advisor and committee is paramount during your academic journey.  Your faculty advisor, graduate program director and graduate program coordinator are also here to support and guide you and we are happy to.  Should any complications arise, don’t wait.  Finding your essential resources are outlined below.  However, please do ask if you do have additional questions.  The CSUS Graduate Handbook is your guide.  Achieving your academic goals is what we are here to help you accomplish!

 

Required Report Forms

Work with your academic advisor on these.  Completed forms must be submitted to the grad program coordinator (GPC).  The current PDF forms should be used at this time.  Formatting issues can be addressed by contacting the GPC.

  • CSUS Annual Assessment Form (all MS & PhD students, annually)
  • MS-A Thesis Proposal
  • MS-A Final Thesis Exam
  • MS-B Project Proposal
  • MS-B Final Project Exam

MS Defense (Plan A)
Submit at least two weeks prior to defense date

PhD Comprehensive Exam
Submit at least two weeks prior to exam date

PhD Defense
Submit at least two weeks prior to defense date

Electronic Thesis and Dissertation Submission (ETD) (MS Plan-B & PhD)

ETD Walk-in Sessions and Formatting Workshops

ETD Deadline Dates
You must submit your ETD for review by the first date, and then all corrections and the final document must be accepted and delivered by the second date.  Visit link for full details.
