MSU Extension Organizational Development

Below you will find civil rights compliance resources for staff who provide programs to youth.  These resources will assist in collecting demographic data, reporting on demographic data that is collected and suggestions on explaining why we collect the demographic data.  These resources are intended for use with online and face to face programs.

• MSU Extension Youth GROUP Demographic Data Collection Form. This form can be used to collect demographic information from groups of youth (i.e., school classroom, soccer club, etc.

• MSU Extension Youth Demographic Data Collection Script Face-to-Face Programs. This file provides a “script” or suggestions for requesting demographic information from youth program participants during face-to-face programs.

• MSU Extension Youth Demographic Data Collection Form Script for Online Programs. 

  This file provides a “script” or suggestions for requesting demographic information from youth program participants during virtual programs.

• MSU Extension Youth Demographic Data Collection Form SPANISH. 

  This form is to assist staff in collecting demographic information from youth participants translated into the Spanish language.

• MSU Extension Youth Demographic Data Collection Form. This form is to assist staff in collecting demographic information from youth participants.

• 4-H Online Crosswalk Youth Demographic. This file can help staff organize and report demographic data into appropriate reporting system.

• 4-H Club Civil Rights Compliance Statement. For compliance purposes, all 4-H clubs are required to fill out this form containing our organizational civil rights statement and their agreement to uphold this commitment. 
•  4-H Civil Rights Review Tip Sheet. This file helps build the awareness of staff in the Children and Youth Institute about civil rights rules, policies, procedures and expectations in their roles and programs.
